Directions:  Simply print out this page, fill it out, and bring it to Silly Munchkins along with the items you wish to consign.  No need to wait while we go through it - just come back within three days if you want to pick up any "no thank you" items.

Thank you for bringing your items to Silly Munchkins.  There may be items we cannot sell and therefore we will decline to accept them for consignment. In the event we have to say “No thank you,” for some of your items, please select one of the following options.

□ Please donate to local charities benefiting children

□ I will return within 3 days to retrieve these items or will forfeit ownership to Silly Munchkin’s for donation.

Please feel free to call us at 247-4559 (24-SILLY) to see if there are any “no thank yous.”  An itemized receipt of your items will be emailed to you, generally within 48 hours.  The consignment period is 60 days.  If you would like to retrieve any unsold items at the end of 60 days, you must do so during the last week of the consignment period.  Call ahead and we’ll have your items ready for you.
